We believe the best care starts before a diagnosis. That's why Poverello runs prevention programs aimed at stopping serious health conditions before they take hold.
Getting 2 Zero is our HIV prevention initiative, built around a simple idea: the more people who know their status and stay in care, the fewer new infections there are in our community. Through outreach, incentives, and one-on-one support, we help people who aren't currently in HIV care get connected and stay connected.
Prevent T2 is our diabetes prevention program. It's a CDC-recognized, year-long lifestyle change program for people at risk of developing type 2 diabetes. Participants work with trained coaches on nutrition, physical activity, and stress management, proven strategies that can cut diabetes risk significantly.
Navigating HIV care can be overwhelming, especially for someone who has been newly diagnosed or is returning to care. Poverello uses trained team members, including individuals with lived experience, to help clients understand their options and connect with the services they need.
Our team helps link individuals to medical care, mental health services, substance-use services, medical and non-medical case management, housing resources, legal assistance and other wraparound services.
The goal is not simply to provide a referral. We help individuals navigate the system, overcome barriers to care and remain connected to the services that support their long-term health and well-being.
